Falls are not a normal part of aging, and are in fact the number one cause of fatal injuries among older adults.
At Age Well, we institute a falls risk assessment at the onset of care, and work to help older Vermonters prevent falls every day.
After every Meals on Wheels referral, an Age Well Wellness Benefits Counselor visits the client to talk about benefits they may be eligible for, perform a depression screening and conduct a falls risk assessment.
The results of the screenings and risk assessment are sent to the client’s primary care provider. If you score high on the falls risk assessment, the Wellness Benefits Counselor may suggest seeking home care support and go over available options.
