Options Counseling

Options Counseling is a short-term, client-directed service that aims to establish and achieve the goals of individuals over 60 years of age and their families.

Age Well’s Options Counselors are experts in all area services, benefits, and resources. Options Counseling can provide the information you need to understand your options and the support to make the best choices for short-term assistance and future care needs.

Options Counseling can help you:

  • Identify your needs and preferences
  • Help families and caregivers understand the range of care settings, services, and support programs
  • Provide information and options for your current or future care needs
  • Facilitate family conversations about ongoing and future care needs
  • Help you to develop a plan as you explore options and consider the benefits and limitations of each choice
  • Make referrals to services you choose, and when needed, connect you with a Care & Service Coordinator who can provide ongoing support and follow up
