Falls Prevention & Risk Mitigation

Falling is not a part of normal aging; every adult has the power to prevent falls. At Age Well, we work to help older Vermonters prevent falls every day.Ronnie OHP

According to Debra Gaylord, Director of Care and Service Coordination, “Falls are the number one cause of fatal injuries among older adults. Instituting a falls risk assessment at the onset of care saves lives.”

After every Meals on Wheels referral, an Age Well Wellness Benefits Counselor visits the client to talk about benefits they may be eligible for, perform a depression screening and conduct a falls risk assessment.

The results of the screenings and risk assessment are sent to the client’s primary care provider. If you score high on the falls risk assessment, the Wellness Benefits Counselor may suggest seeking home care support and go over available options.
