Senior Fall Prevention Programs in Vermont

Falls are not a normal part of aging, and are in fact the number one cause of fatal injuries among older adults, which is why at Age Well fall prevention for seniors is a top priority. We are dedicated to ensuring the safety and well-being of older adults in our community, which is why we have implemented comprehensive fall prevention programs and services aimed at reducing the risk of falls among older Vermonters.

Our approach begins with a thorough home fall risk assessment at the onset of care, allowing us to identify potential hazards and tailor fall interventions to each individual’s needs. Through education, exercise programs, home modifications, and community resources, we empower older adults to take proactive steps in preventing falls and maintaining their independence. Our team of dedicated professionals is committed to promoting a safe and supportive environment for older adults to thrive in Vermont.

Fall Risk Assessments for Older Adults

After every Meals on Wheels referral, an Age Well Wellness Benefits Counselor visits the client to talk about benefits they may be eligible for, performs a depression screening, and conduct a fall risk assessment.

The results of the screenings and risk assessment are sent to the client’s primary care provider. If you score high on the fall risk assessment, the Wellness Benefits Counselor may suggest seeking home care support and go over available options.

Falls Free Vermont

Age Well is an active member of Falls Free Vermont, a coalition of organizations and individuals with a mission to reduce preventable falls and fall-related injuries and deaths in older adults. At Falls Free Vermont, they strive to meet our mission through the following goals and activities:

  • Provide statewide resources
  • Raise awareness
  • Increase education & training

Fall Prevention Methods & Education

Age Well offers evidence-based fall prevention programs for older adults like Tai Chi to help reduce stiffness and pain and keep joints mobile. Taught by certified trainers, there is no charge for these classes, but donations are gratefully received.
