Veteran Directed Care is a self-directed nursing home diversion program that allows Veterans to hire private individuals to provide their personal care.
Hosted by the U.S. Department of Veterans Affairs, Veterans Health Administrative, White River Junction Healthcare System in partnership with: Age Well, ARIS Solutions, Senior Solutions, Central Vermont Council on Aging, Northeast Kingdom Council on Aging, and Southwestern Vermont Council on Aging.
