Diabetes Self-Management Program

This free evidenced based workshop will be a Zoom meeting, join us for a introductory session on June 5th to meet the facilitators and other participants. The next six weeks we will meet for 2 1/2 hours covering a variety of self-management tools and resources that can help you best manage your health.
Your workshop leaders and participants will work together to guide and support one another to:
- Solve problems and create plans for action
- Make healthier eating choices
- Get more physical activity
- Better monitor your blood sugar levels
- Find ways to manage your stress and more!
Our workshops are free of cost for all participants. That’s because we’re committed to making sure all Vermonters* have access to expert advice and proven strategies to take control of their health.
*New York and New Hampshire residents who have a Vermont health care professional may qualify!
