Good nutrition plays a vital role in healthy aging. As we grow older, our bodies require balanced, nutrient-dense meals to support immunity, maintain strength, manage chronic conditions, and promote overall well-being. Yet many older adults face challenges that make meal preparation difficult—whether due to declining health, mobility limitations, or the demands of managing complex health concerns.

That’s why Age Well is committed to ensuring that every older Vermonter has access to meals that are not only healthy and delicious but also tailored to support individual medical and dietary needs.

Through Meals on Wheels and our therapeutic meal program, we provide dependable nutrition and home-delivered meals for seniors that empower older Vermonters to stay healthy, safe, and independent at home.

Home-Delivered Meals That Support Independence

Age Well is pleased to offer a variety of healthy home-delivered meal options for seniors in the Chittenden, Franklin, Addison, and Grand Isle service area. These meals are designed to support individuals in managing chronic health conditions safely at home. Meals on Wheels provides one meal per day that meets one-third of the Dietary Reference Intake (DRI) for adults over age 60, as required by the Older Americans Act (OAA). 

Our goal is to provide meals that are delicious, nutritious, and easy to reheat and enjoy, especially for clients who may be experiencing temporary or long-term health challenges. Eligibility for the program is clearly defined, and Age Well intake staff are available to help individuals determine whether they qualify. Meals are partially funded through the OAA, allowing us to serve older adults who are unable to prepare their own meals due to health-related limitations.

Expanding Therapeutic Meal Options to Meet Individual Needs

In 2018, Age Well streamlined the therapeutic meal process and began offering a wider variety of personalized nutrition options. In 2023, our work was recognized nationally by USAging, underscoring our leadership in providing specialized nutrition through home-delivered meals for seniors. 

Each therapeutic meal includes: 

  • A good source of protein 
  • Three servings of fruits and/or vegetables 
  • Grains that provide essential nutrients 
  • A serving of calcium-rich milk. 

Please note: Therapeutic meals may not follow the standard monthly menu, as items are adjusted or substituted based on dietary requirements. We proudly offer the following therapeutic diets, and can combine options to meet unique client needs.

Home-Delivered Therapeutic Meal Options for Seniors

Our therapeutic meal options are thoughtfully prepared to support a variety of health needs without sacrificing taste or enjoyment. Each meal follows trusted nutrition guidelines and is designed to help manage specific conditions while providing balanced, satisfying options you can feel good about choosing.

Heart Healthy Meals

Designed for individuals managing heart disease, hypertension (high blood pressure), and hypercholesterolemia (high cholesterol).

Each meal contains: 

  • Less than 800 mg sodium 
  • Less than 15 g fat 
  • Minimal trans fats

Diabetic-Friendly Meals

For individuals managing diabetes. 

Each meal provides: 

  • Consistent carbohydrate servings 
  • No more than 75 g of carbohydrates per meal

Renal-Friendly Meals

For clients diagnosed with renal disease or undergoing related treatments.

Meals include: 

  • Less than 1,000 mg sodium 
  • Less than 1,000 mg potassium 
  • No more than 240 cc of fluids, including milk

Lactose-Free Meals

For individuals with lactose intolerance or allergies.

Prepared without: 

  • Milk
  • Cheese 
  • Other dairy products
  • Lactose-free alternatives (milk, cheese, and similar products) may be provided as appropriate.

Gluten-Free Meals

For individuals with gluten intolerance or allergies. 

Meals contain no:

  • Wheat
  • Barley
  • Rye
  • Or gluten-containing ingredients

Vegetarian (Lacto-Ovo) Meals

This option supports adequate protein intake while offering a varied vegetarian menu.

Includes: 

  • Milk 
  • Cheese 
  • Eggs 

Texture-Modified Meals

For clients experiencing: Difficulty chewing, dental issues, swallowing challenges, general weakness, or fatigue affecting eating.

We provide:

  • Mechanical Soft Meals—fork-tender foods and chopped meats
  • Pureed Meals—smooth, consistent textures similar to oatmeal
