Keynote

 Integrating Health & Social Services: How can the relationship between hospital and community partners combat rising healthcare costs, prevent emergency room visits, ensure prescriptions are being filled and properly taken, fight against chronic disease and support healthy aging?  How can organizations like Age Well be an integral and proactive piece of the healthcare puzzle?  Few people in healthcare question the role that social needs play in one’s health.  Dr. Michael LaMantia will discuss the growing interest, momentum, and possibility of putting this understanding into practice for providers, including how we can effectively move the dial towards creating an integrated model.

Dr. Michael LaMantia is a geriatrician and section head of geriatric medicine at The University of Vermont Medical Center. He is also an associate professor at the UVM College of Medicine in Burlington, VT. Dr. LaMantia specializes in the care of older adults, particularly those with cognitive impairment including memory and thinking skills. His research focuses on the coordination of care for older, vulnerable patients as they transition across sites of health care delivery. He has a particular interest in the care of seniors in the emergency department and especially the care provided to seniors with delirium and dementia. Dr. LaMantia considers discussing his patients’ health and challenges a privilege and is honored to share in their health care decisions. Learn more about elder care at the UVM Medical Center.
