Choices for Care: Exploring Long-Term Care Options for Older Adults

 As a Choices for Care provider,  Age Well can help you explore long-term care options. By coordinating a customized, in-home visit, we can connect you with the support and services to assist with everyday activities and make informed decisions about how you would like to age.

What is Choices for Care?  Vermont’s Choices for Care is statewide initiative that began in 2005 to help individuals stay at home, where they want to be. By placing more emphasis on home and community resources and less on institutional care, it gives individuals more choice in where and how they receive long-term care while granting equal access to available services.

Choices for Care is a Medicaid-funded, long-term care program that pays for care and support for older Vermonters and people with physical disabilities. Essentially, it helps individuals carry out everyday activities at home, in an enhanced residential care setting, or in a nursing facility.

For example, if you live at home, Choices for Care can pay for you to receive help with medication, transportation, housekeeping, meal preparation, laundry, and more. Choices for Care can also help pay for respite care, adult day services, and companion care as well as services and support if you are in a nursing home or residential care facility.

Am I eligible?  To become a recipient of Choices for Care, you must:

  • Be a Vermont resident,
  • Be at least 65 years old OR at least 18 years old with a physical disability,
  • Meet the financial criteria, and
  • Meet the clinical criteria for nursing home level of care.

What is Age Well’s role as your Choices for Care provider? Age Well will connect you with one of our Care Coordinators, who will assist you with gaining access to medical, social, financial and other supports. To establish a continuum of care, your Care Coordinator will meet with you wherever you are and complete an initial assessment. This meeting will determine the number of hours of care necessary and services available to you through Choices for Care. Once a plan is in place, we will coordinate services and monitor any changes in your circumstances to help you remain independent for as long as possible.
